In Washington, teens ages 13 to 17 can consent to their own mental health and substance use treatment, and they control who can see the records of that treatment. If you have questions about a teen’s records, please call our office.
A personal representative of a client who has passed away may request records with proof of authority.
Requests from courts, attorneys, and agencies
Subpoenas, court orders, and requests from agencies are reviewed before any records are released, as required by state and federal law. Please send them by mail or fax.
Your privacy
Records are released only as allowed by state and federal law, including HIPAA, Washington’s Uniform Health Care Information Act, and, for substance use treatment records, 42 CFR Part 2.
Please do not send health information through this website’s contact form or chat.
