COMMUNITY ACCESS TEAM

Transforming Access to Care.

United Family Center’s Community Access Team (CAT) aims to provide mobile and accessible behavioral health services that supports the mental and emotional well-being of youth and their families within community settings. 

All CAT services are conducted in environments that are safe and accessible to community members, such as schools and community centers,  and are delivered by licensed and degree-level professionals with the experience and compassion needed to care for individuals in vulnerable situations.  

SERVICES

BEHAVIORAL HEALTH COUNSELING

Provided to manage behavior and emotional well-being. These services are accessible directly within community settings, making it easier for clients to receive care without significant disruption to their daily lives.

MENTAL HEALTH
SERVICES

General services to promote mental health and well-being through supportive interventions like stress management workshops, mindfulness sessions, and educational seminars.

FAMILY & COUPLES THERAPY

Therapy sessions that focus on resolving interpersonal conflicts and improving communication within couples and families.

MULTI-FAMILY GROUP PSYCHOTHERAPY

Community-based group sessions that involve multiple families. These sessions are designed to foster a community support network and provide a platform for shared experiences and mutual support.

PSYCHOTHERAPY GROUPS

Group therapy sessions focused on addressing specific mental health issues such as anxiety, depression, and PTSD, held in various community venues.

CRISIS INTERVENTION

Immediate, on-the-spot assistance for acute behavioral crises. This rapid response helps manage emergency situations where mental health support is urgently needed.

COMPREHENSIVE COMMUNITY SUPPORT

Broad support services that include life skills training, employment assistance, and community involvement programs to facilitate integration and selfsufficiency.

SUBSTANCE USE
DISORDER

Tailored management plans designed for individuals dealing with substance use challenges, focusing on recovery and relapse prevention

INDIVIDUAL & GROUP
THERAPY

Targeted therapy sessions for different age groups (adults and youth) and family roles, designed to address specific life challenges and developmental issues.

FAMILY COUNSELING

Therapy that focuses on improving the interactions and overall dynamics within a family unit.

CASE MANAGEMENT

A comprehensive approach to managing the various aspects of a client’s care, from initial assessment through to the coordination of services and follow-up.

URINE ANALYSIS (UA)

Drug screening tests conducted as part of the ongoing treatment for substance use disorders.

ACCESS

Our Community Access Team will deliver services right in your neighborhood – at community centers, schools, and other accessible locations, ensuring everyone has easy access to the help they need. We follow a referral-based process to ensure the individual needs of willing participants are cared for appropriately.

REFERRAL

Our team receives referral from school staff, parent, or youth.

INITIAL CONTACT

Contact is made with youth and/or parents to gather basic information.

CONSENT

Written consent must be received from parents (and youth if appropriate) before moving forward.

CONSULTATION

Conduct assessment to decide treatment plan and steps forward.

TREATMENT

Services are delivered based on the treatment plan created. Treatment is a multi-step process which includes:

  • Counseling 

  • Regular monitoring of progress

  • Collaboration with relevant outside groups, such as school staff

  • Parental Engagement for youth and family services

  • Escalation or Referral for higher level care if progress stalls or more complex needs arise

If and when counseling goals are met, or if the individual no longer requires the service, we will formally close out the counseling process; counselor may recommend future care. If necessary, we will arrange for follow-up to ensure continuity of care or to check on the individual's well-being post-counseling.
